The name of applicant organization shall be the same as that listed in the accreditation certificate issued by the Taiwan Accreditation Foundation or accreditation bodies that are members of the Multilateral Recognition Arrangements of the International Accreditation Forum or the Pacific Accreditation Forum.

The application scope of recognition shall be limited to those covered in the CNS Mark product items as announced by BSMI and to those covered in the accreditation scope of the TAF.

Foreign applicant may fill out a Letter of Authorization to authorize an agent that has a domestic domicile or business place to make the application on his behalf.

Where the applicant has become a recognized quality management system certification body under the BSMI Registration of Product Certification Scheme, the documents mentioned in items 4 and 5 of the application form are not required.

The person in charge of the applicant certification body shall sign his signature, instead of typing, in the application form and indorse the company seal.
